Inclusive Communities

Inclusive Communities is a community-led project designed to improve access to specialised rehabilitation and disability support for people living with brain injury and other complex disabilities across Far North and North West Queensland.

The project was created in response to longstanding challenges identified by local communities, including fragmented services, workforce shortages, geographic barriers and difficulties navigating support systems. Unlike traditional service models, Inclusive Communities is co-designed with people with lived experience, families, local providers and rehabilitation specialists. It takes a place-based approach, supporting people to achieve their own goals — whether that’s reconnecting with friends, returning to work, participating in sport or engaging in community life — while strengthening the capability of local services to deliver coordinated, culturally responsive support close to home.
